Between my undergraduate and graduate degrees, I took on a role as a Nursing Assistant (NA), aiming to gain hands-on clinical experience and insight into patient care. In that role, I had the opportunity to work closely with patients who faced a wide range of challenges. Many were dealing not only with medical conditions but also with barriers like limited access to transportation, unstable housing, or food insecurity. I saw firsthand how these social factors had a profound impact on their health and well-being—and how often they went unaddressed.

 

This experience deepened my understanding of why healthcare must go beyond clinical treatment and address the full spectrum of social and environmental factors. It also underscored the importance of community health workers (CHWs), who are uniquely equipped to connect with residents, address health disparities, and provide continuity of care where traditional healthcare models often fall short. These observations ultimately fueled my passion for supporting community health at a systems level—leading me to join Continual Care Solutions.

 

At Continual Care Solutions, our mission is to equip healthcare networks and community-based organizations with the tools they need to provide coordinated, person-centered care. We’re thrilled to announce that our platform, IMPOWR, recently earned provisional certification as a Pathways Community HUB Institute (PCHI) vendor, which underscores our commitment to high standards in community-based care coordination. Our platform now aligns with the PCHI Model, a framework that enables CHWs to systematically address both medical and social determinants of health across 21 Standard Pathways, from housing stability to employment and beyond.

 

Our platform empowers CHWs by simplifying these processes with features like intuitive dashboards, standardized reporting tools, and client tracking capabilities that enhance both efficiency and accuracy. This means CHWs can focus on what truly matters: engaging with community members, building trust, and making measurable improvements in health outcomes. By helping CHWs track client progress from risk assessment to outcome, our platform supports a transparent, accountable approach to healthcare that aligns resources and fosters health equity.
